Offering one-on-one guidance and connections to support services, Safe Place for Youth’s case managers develop individualized plans that address short and long-term goals towards stability. This includes housing navigation, applying for identification documents and health benefits, transportation assistance, parental and prenatal care, family reunification, substance use rehabilitation, mental health services, and employment and education support.
Through a trauma-informed approach, our clinical team provides holistic case management to meet the physical and psychosocial needs of our vulnerable youth.
